The death of an individual in Poland, regardless of citizenship or nationality, requires the completion of the relevant formalities.
The first document needed to register a death is a death card. It is issued by a doctor who certifies the death of a person. If the death did not occur in a hospital or medical facility, an ambulance must be called. The death can be declared by a doctor or the head of the emergency medical team.
The death should be reported to the registry office. The notification should be made within three days of the issuance of the death card, and in the case of death due to an infectious disease – within 24 hours. The document confirming the death is the death certificate, issued by the Head of the Registry Office having jurisdiction over the place of death of the person.
